TUTORIAL SERVICES CONTRACT

PLEASE READ BEFORE SIGNING

As a tutee in the Mount Aloysius College Tutorial Program, I understand my responsibilities are as follows:                             

1.  I will attend all classes for which I request tutoring. If I cannot attend a class, I will notify the appropriate instructor as soon as possible and make arrangements to make up any and all missed work.

2.  I will attend all tutorial sessions as scheduled. If I cannot keep my scheduled appointment, I will notify the appropriate tutor within 24 hours. If I miss more than 3 scheduled tutoring sessions without formal notification, I may be dropped from the program.

3. I will come to the tutorial sessions prepared with specific questions, problems, books, syllabus, tests, papers, and concerns for my tutor.

4. I will be permitted no more than two hours of tutoring per course per week. If necessary, I will request additional hours in writing to the Tutorial Coordinator. Permission will be granted based on tutor availability.

I understand that if I do not meet the obligations as stated in this contract, tutorial services may be withdrawn for the duration of the semester in which the violation(s) occurs.
